Frank's father has given her daily doses of testosterone in her food. To avoid the heightened sexual arousal that this would cause (if she had become aroused she might have found her clitoris) he has also dosed her with bromide. Effectively Frank is castrated because she has no access to her authentic sexuality. She has been castrated by her father. Remarkably when she does discover the truth she immediately embraces it, by respelling her name, Frances Lesley Cauldhame not Francis Leslie.
